PGAD or Persistent Genital Arousal Disorder, what is it?

PGAD is described as unwanted, sensations in the genital an/or anal regions, including arousal, tingling, itching, burning, swelling, changes in lubrication, to name a few. It may cause unwanted and not be relieved by orgasm. It may last for extended periods of time. 


Studies thus far, show it is more prevalent in females or those assigned female at birth, but can occur in males or those assigned male at birth.


The cause is not clearly defined. It may be linked to nerve irritation in the pelvis or spine, cysts in the pelvis, anxiety or other mental health disorders, physical trauma to the pelvis, and/or excessive pelvic floor muscle tension.


Treatment for PGAD often begins with being evaluated by a group of medical practitioners (one or more) such as gynecologists, urologists, pelvic pain physicians and/or neurologists.  


Certain medications that help with nerve modulation can be helpful. Pelvic physical therapy can be extremely helpful. Physical therapy techniques can help retrain pelvic and core muscles and quiet the nervous system.  


Biofeedback therapy utilizing manual physical therapy techniques or computer driven training helps to reduce tension and promote healthy coordination of pelvic muscles. Meditation, along with mindfulness and breathing techniques can also be part of a treatment program. 


 At Lotus Physical Therapy for Women, along with the aforementioned treatment options, I also incorporate gentle movement re-education. All of these combined techniques help to teach muscles, nerves and the brain new patterns of behavior which can help to decrease the discomfort from PGAD. 


 For best outcomes, I also work closely with the patient's referring physician to ensure the most comprehensive and coordinated care.
